HEALTH SURVEY IN BOLLINGER COUNTY

One thousand residents from Bollinger County have been randomly selected to participate in the Bollinger County Health Assessment Survey.

This survey is being conducted by the Community Health Assessment Resource Team (CHART) of Bollinger County. The survey data will be analyzed and used as a tool to assess and prioritize community health problems.

The need for additional health care is great in Bollinger County and this survey will help to determine those needs. In order for the survey to be effective, the information that is received back must be completed.
